Edit: Looking back at an archived forum post, this apparently happened to some realms in 2011 during the Cataclysm expansion. So, this exact issue has happened before and many players suspected then it was the result of hacking. Thanks to Hathorne for the Tip!
Frostmourne is the weapon carried by The Lich King in Icecrown Citadel.
